Preparing for your Appointment
Please refer to your insurance booklet, and please bring all insurance information with you and our staff will assist you in any way they can. If your insurance requires a referral, contact your primary doctor for a referral prior to scheduling an appointment.
Co-pays are due at the time of visit. For your convenience, we accept Visa, MasterCard, Discover and American Express.
Parking and Transportation
You’ll find easy-access parking with first floor access to our South Euclid and Rocky River offices. Both offices are located at street level. A Regional Transit Authority (RTA) stop is located close to both locations.
The highest compliment our patients can give is a referral of their family and friends. We will be glad to help them in any way we can.
